Root Cause Audit
Diagnosis before treatment. In one to two weeks you know the cause — not the culprit — and the plan that follows from it.
The typical symptoms
- Releases stopped, or every release breaks something new.
- The vendor’s status reports are green; the product visibly is not.
- Performance collapses under load nobody considered unusual.
- The team knows something is wrong and cannot agree on what.
- Money is burning and the explanations keep changing.
The process
One to two weeks, fixed. We read the code, the infrastructure, the tickets, and the delivery history; we interview the people closest to the failure — engineers first, management second. Then we reconstruct the chain: not the moment things broke, but the decision that made breaking inevitable.
Cause, not culprit
Crisis reviews usually turn into blame allocation, which is why they usually fix nothing. Our report names mechanisms: the architectural choice, the missing feedback loop, the incentive that rewarded the wrong thing. People problems appear as structures, because that is what they almost always are.
